We have an ISA 2006 server sitting in the DMZ network, CAS server is behind
in the internal network. What ports are required between CAS and ISA
servers for RPC over HTTPS to work properly? At this point, clients can't
connect from outside the LAN using RPC over HTTPS. OWA works fine though.

Firewall allows port 80 and 443 traffic from Internet.

You should only need port 443...

If you open Outlook using outlook.exe /rpcdiag what do you see? Also, can
you browse to the external FQDN of your CAS server from inside the network?
You should be able to resolve to http://mail.yourdomain.com/owa inside your
network.....

When using rpcdiag I see the attempt to establish 3 types of connections
using https - referral, mail and directory.

You see the referral come and then go then it tries mail and directory, mail
eventually fails and then it establishes 2 directory connections using https
successfully - I think it is the mail connection that I need to get it right
as well as the directory.

When trying http://mail.mydomain.biz/owa it requests that you use https - if
I try it with https page can not be found. I can use owa internally and
externally using https://mail.mydomain.com/exchange
