On Windows Server 2008 I install Microsoft Exchange Server MAPI Client and
Collaboration Data Objects 1.2.1
(http://www.microsoft.com/downloads/details.aspx?familyid=E17E7F31-079A-43A9-BFF2-0A110307611E&displaylang=en).
When I attempt to run MAPI Editor
(http://www.microsoft.com/downloads/details.aspx?FamilyID=55fdffd7-1878-4637-9808-1e21abb3ae37&DisplayLang=en)
I receive a MAPI_E_CALL_FAILED error when MAPIInitialize
(http://msdn2.microsoft.com/en-us/library/ms527051(EXCHG.10).aspx) is
called. I have attempted to use on both 32 and 64 bit versions of the
Windows Server 2008 operating system and both fail.

Has anyone been successful making the stand alone MAPI/CDO operate on
Windows Server 2008?

Try to run the process explorer to see if MFCMAPI is picking up the right
mapi32.dll.
This error is usually an indication that you are picking up the mapi stub
dll.
